I received a call from the vet yesterday about Boris' urinalysis and BP test from Friday. Proteinuria continues and BP is high, so the vet has decided to prescribe Telmisarten, which should address the high BP as well as the proteinuria. I asked about the increasing BUN with decreasing creatinine, and she said that so many factors drive those numbers that it wasn't as important as the numbers being high overall. Personally I'm wondering if the change from Dr. Elsey's to Young Again may have had a minor effect, but only time will tell.
This morning I noticed that his protein level was 100 back in June, but is at 2+ as of Friday. It appears to me that is a significant reduction, but I don't really understand how protein is measured. Do different labs use different scales? Is that number coming down? If so, do we really NEED the Telmisarten? I don't understand the difference between acute and chronic KD either, so I want to make sure we're actually dealing with CKD and not another issue that is being missed. Our lifestyle results in bloodwork from different sources and can make spotting trends more difficult.
Boris' appetite is still unpredictable. Discussed Cerenia with the vet, and she agreed we can use the remaining Cerenia supply from our IBD cat to see if we can get him eating. After a dose of Cerenia he did eat all of his dry and a small portion of his wet food overnight. I don't know if the food change is part of the problem with eating, so this morning I gave just Young Again since he is eating dry more readily than wet. I know we can't continue on just dry indefinitely, but administering insulin is almost impossible right now since his appetite is so unpredictable. I'm grateful that his BG is staying in the 100's despite the erratic insulin schedule! My hope is that he'll be more interested in the wet food tonight or tomorrow.
The vet is concerned that he might also have an infection brewing, so I am going to take him today for a Convenia shot just in case. My schedule is ridiculous right now, so while I am there I am going to ask for Mirtazapine to see if we can stimulate his appetite a bit to get him eating more reliably. I'm hoping that will help us push through the wet food change while we get his nausea under control.
Boris has an appointment with a larger veterinary hospital's pain management clinic next Monday to see what we can do to address the arthritis pain in the last 1/2 - 1/3 of the month as his Solensia wears off. I confirmed they administer PRP Injections to cats, but they said there are many other avenues to explore as well. Fingers crossed we can at least get his pain levels under control. My gut feeling is he may not need insulin if we can control his pain; I'm seeing a pattern of reduced BG after Solensia, then his BG creeping back up as the pain comes back.
